Would you like to provide input into a new Fraser Health Cardiac Check-In pilot program at Royal Columbian Hospital?
- Have you received medical care at the Royal Columbian Hospital within the past two years for a heart attack or angioplasty?
- Do you live in the Fraser Health area?
- You will be asked for your comments and impressions of this new service.
Purpose
This new post discharge service is intended to support patients during their first 4 weeks after discharge.

Tuesday December 6 from 1:30 to 2:30 pm
Location
Royal Columbian Hospital - 1330-1430 at RCH Cardiac Clinic, ground floor in the Columbia Tower
Background
Before the meeting, an orientation document will be provided for your review
Follow-Up
You will be invited to a follow-up meeting(s) after the start of the pilot project for a discussion and progress updates.
