Patient opportunity: Review & Redesign Diabetes Teaching and Treatment Centre

Patient Voices Network has an opportunity for 2 members that have attended our orientation session, live in the lower mainland, are living with diabetes, and have received some form of diabetes care, or received self-management support for diabetes.

Background:

A Health Authority is looking to review and redesign the Diabetes Teaching and Treatment Centre (DTTC) for a hospital.  The new vision for the Diabetes Teaching and Treatment Centre will:

  • Be based on current best practices
  • Optimally serve the needs of the population
  • Provide service in an effective and efficient manner

The future state of the Diabetes Teaching and Treatment Centre will complement:

  • Provincial strategies for diabetes management
  • Services available in the lower mainland
  • Providence Health’s mission, vision and values

Opportunity:

Two Patient Partners are invited to participate in two meetings held at a hospital in Vancouver. 

Our hope is that the volunteers will:

  • Fully participate in discussions by providing input, feedback and asking questions
  • Help to make sure the group’s thinking is truly patient centered

 DETAILS

Meeting #1 (4 hours):  Tuesday, February 28 from 1pm – 5pm

  • Facilitated discussion
  • Small-group exercises
  • Creative thinking & visioning for the future state of the Diabetes Teaching & Treatment Centre

 Meeting #2 (2 hours):  Date & Time to be determined (based on availability of participants)

  • Proposed care model based on information from meeting #1
  • Evaluation & Feedback on proposed model

Location: Vancouver (exact location to be confirmed)

Transportation: PVN encourages taking public transit as parking in the area can be challenging.  If you do drive, please allow time to find parking as the parkade at St. Paul’s is often full at that time.  PVN can provide transit passes with advance notice.  Parking information and directions to the meeting space will be provided separately.

Expenses: Parking or transit fare will be reimbursed for this meeting. Please submit original receipts to PVN.  Mileage will not be reimbursed.
